摘要:
心外膜脂肪组织(EAT)是心肌和心外膜内脏层之间的独特脂肪库,与心房颤动(AF)关系密切。近年来,研究发现EAT不仅具有活跃的内分泌和旁分泌功能,还聚集分布着交感和副交感神经纤维。其异常堆积与AF风险显著相关。目前,EAT与自主神经之间的交互机制尚未完全明确。本文系统综述了EAT如何通过脂肪浸润、神经节丛调控以及旁分泌作用与神经系统相互作用,从而促进AF的发生。目前研究发现部分药物通过代谢调节靶向EAT,未来研究需构建精准评估体系,并探索新型治疗策略,为AF防治开辟新方向。
Abstract:
Epicardial adipose tissue (EAT) is a unique fat depot between the myocardium and the visceral layer of the epicardium,which is closely related to atrial fibrillation (AF). In recent years,it has been found that EAT not only has active endocrine and paracrine functions,but also accumulates and distributes sympathetic and parasympathetic nerve fibers. Its abnormal accumulation is significantly associated with the risk of AF. Currently,the interaction mechanism between EAT and autonomic nerves has not been fully clarified. In this paper,we systematically review how EAT interacts with the nervous system through fat infiltration,ganglionic plexus modulation,and paracrine effects to promote AF. The current study found that some drugs target EAT through metabolic regulation,and future research needs to build a precise assessment system and explore novel therapeutic strategies to open up new directions for AF prevention and treatment
