摘要:
目的 探讨高血压患者心肌微循环损伤与运动试验中血压、心率变化的关系。方法 选取住院的高血压患者74例,根据心肌声学造影实时评估心肌微循环灌注,分为微循环显像正常组和异常组。搜集研究对象一般临床资料、生化指标和平板运动参数等指标。比较两组患者各指标间的差异,分析高血压患者心肌微循环与运动后血压、心率的关系。结果 (1)高血压心肌微循环异常组与正常组比较:中心动脉收缩压升高,运动性高血压患病率增加;静息心率、峰值心率、心率储备和平均心率均减小;运动峰值收缩压及舒张压升高、代谢当量下降,以上差异均有统计学意义(P<0.05)。(2)运动性高血压、峰值心率和中心动脉收缩压是高血压患者心肌微循环损伤的独立危险因素。结论 高血压患者常伴有运动性高血压反应;峰值心率减慢、中心动脉收缩压增高增加心肌微循环损伤风险。
Abstract:
Objective To explore the relationship of myocardial microcirculation injury with the changes of blood pressure and heart rate during exercise test in hypertensive patients. Methods A total of 74 hospitalized hypertensive patients were enrolled,and divided into normal and abnormal microcirculation groups(NM and AM groups) according to myocardial microcirculation perfusion r eal-time assessed by myocardial contrast echocardiography . The general clinical data,biochemical indicators,treadmill exercise parameters and other indicators were collected. Differences between two groups were compared ,and the relationship of myocardial microcirculation injury with blood pressure and heart rate after exercise were analyzed . Results ( 1) Compared with NM group,the central aortic systolic blood pressure(CSBP) and prevalence rate of exercise hypertension in AM group were increased with resting heart rate,peak heart rate,heart rate reserve,average heart rate decreased,peak systolic and diastolic pressure elevated and metabolic equivalent dec lined. The above differences were all statistically significant (P<0.05). (2) Exercise hypertension ,peak heart rate,CSBP were independent risk factor s for myocardial microcirculation injury in hypertensive patients. Conclusion Hypertensive patients often possess exercise hypertension and decreased peak heart rate ,while elevated CSBP may increase the risk for myocardial microcirculation injury
