摘要:
目的 系统性评价基于虚拟现实(VR)的心脏康复对冠心病(CHD)患者焦虑、抑郁、压力以及心肺功能等多方面的作用。方法 在Pubmed、Embase、Web of Science、Cochrane Library、中国知网等数据库中使用个体检索策略,以检索从建库截至2025年6月10日发表的同行评审期刊文章。这些文章比较了基于VR的心脏康复与标准心脏康复对CHD患者的心理情绪和心肺功能影响。随后,按照纳入与排除标准筛选文献,采用Cochrane偏倚风险评估工具2.0版本对纳入的文献进行质量评价。最后,采用Stata 15.1软件进行meta分析,连续性变量以标准化均值差(SMD)及其95% CI为效应量。结果 总共检索到366篇文章,其中10项研究被纳入meta分析,涉及530例患者。接受基于VR的心脏康复的患者纳入试验组(n=239),而接受常规心脏康复的患者纳入对照组(n=291)。meta分析结果显示:与常规心脏康复相比,基于VR的心脏康复能够显著降低CHD患者的焦虑评分(SMD=-0.56,95% CI -0.98~-0.14 ,P=0.008)、抑郁评分(SMD=-0.38,95% CI -0.64~-0.13 ,P=0.003)和压力评分(SMD=-0.58,95% CI -0.84~-0.31 ,P<0.001)以及显著提高CHD患者的6分钟步行距离(SMD=1.12,95% CI -0.74~1.50 ,P<0.001)。结论 基于VR的心脏康复能够显著降低CHD患者的焦虑、抑郁和压力评分以及改善CHD患者的心肺功能。
Abstract:
Objective To systematically evaluate the effects of the virtual reality(VR)-based cardiac rehabilitation on various aspects of anxiety,depression,stress,and cardiorespiratory function in patients with coronary heart disease (CHD). Methods Individual search strategies were used in databases such as Pubmed ,Embase,Web of Science,and Cochrane Library in order to retrieve peer-reviewed journal articles published from the build deadline to June 10,2025. These articles compared the effects of the VR-based cardiac rehabilitation with standard cardiac rehabilitation on psycho-emotional and cardiorespiratory function in patients with CHD. Subsequently,the literatures were sceened according to the inclusion and exclusion criteria,and the quality of the included literatures was evaluated using the Cochrane’s risk of bias tool version 2.0. Finally,meta-analysis was performed using Stata15.1 software,and continuous variables were used as effect sizes with standardized mean difference (SMD) and their 95% CI. Results A total of 366 articles were retrieved,of which 10 randomized controlled trial studies were included in the meta-analysis involving 530 patients. Patients who received the VR-based cardiac rehabilitation were included in the experimental group (n=239) while patients who received conventional cardiac rehabilitation were included in the control group (n=291). meta-analysis showed that compared with usual cardiac rehabilitation,the VR-based cardiac rehabilitation was able to significantly reduce anxiety score (SMD=-0.56,95% CI -0.98~-0.14 ,P=0.008),depression score (SMD=-0.38,95% CI -0.64~-0.13 ,P=0.003),and stress score (SMD=-0.58,95% CI -0.84~-0.31 ,P<0.001) as well as a signifcantly increased six minutes walk distance (SMD=1.12,95% CI -0.74~1.50 ,P<0.001)in patients with CHD. Conclusion The VR-based cardiac rehabilitation can significantly reduce anxiety,depression and stress scores and improve cardiopulmonary function in patients with CHD
