摘要:
体外膜肺氧合(ECMO)是在体外循环基础上发展起来的体外生命支持技术,为各种原因导致的重症心肺功能衰竭患者提供暂时性心肺支持,争取原发疾病治疗和恢复的时间窗口,在危重患者的救治中发挥着十分重要的作用。随着婴幼儿心脏手术水平的提高,先天性心脏病(CHD)患儿手术年龄不断降低,手术难度系数也不断增加,CHD术后需要ECMO支持治疗的高危患儿也日益增多。及时有效的ECMO辅助,能维持CHD术后严重心力衰竭患儿循环的稳定,改善组织缺氧状态,进而提高CHD术后高危患儿的生存率。现对ECMO辅助治疗在婴幼儿CHD术后的应用现状、辅助策略、并发症及发展方向等做如下分析总结。
Abstract:
Extracorporeal membrane oxygenation(ECMO) is an extracorporeal life support technology developed on the basis of cardiopulmonary bypass,which provides temporary cardiopulmonary support for patients with severe cardiopulmonary failure caused by various reasons,and strives for the time window for the treatment and recovery of primary diseases,and plays a very important role in the treatment of critically ill patients. With the improvement of the level of infant heart surgery,the operation age of children with congenital heart disease(CHD) is decreasing,the difficulty coefficient of operation is increasing,and the number of high-risk children who need ECMO support treatment after CHD surgery is increasing. Timely and effective ECMO assistance can maintain the circulation stability of children with severe heart failure after CHD surgery,improve the hypoxia state of tissues,and then improve the survival rate of high-risk children after CHD surgery. Therefore, we analyzed and summarized the application status,auxiliary strategies,complications and development direction of ECMO adjuvant therapy in infants with CHD after operation as follows
