摘要:
偏头痛是一种常见的神经系统疾病,而卵圆孔未闭(PFO)作为一种常见的先天性心脏病,与偏头痛的关系受到广泛关注。现综述PFO与偏头痛之间的流行病学关系、潜在病理生理机制以及PFO介入封堵术治疗偏头痛的效果。研究表明,PFO在有先兆偏头痛患者中的发生率显著高于普通人群,且右向左分流的存在可能与偏头痛发作频率和严重程度相关。影像学研究发现,PFO患者更易出现脑白质病变和大脑功能网络异常。尽管PFO介入封堵术在部分病例研究中显示出对偏头痛的潜在疗效,但随机对照试验的结果并不理想,提示其治疗效果仍需进一步验证。
Abstract:
Migraine is a common neurological disorder,and patent foramen ovale(PFO),as a common congenital heart disease,has garnered significant attention for its relationshipwith migraine. This review summarizes the epidemiological associations between PFO and migraine,the underlying pathophysiological mechanisms,and the therapeutic effects of PFO transcatheter closure for migraine. Studies have shown that the prevalence of PFO is significantly higher among patients with migraine with aura compared to the general population,and the presence of right-to-left shunt may be associated with the frequency and severity of migraine attacks. Imaging studies reveal that PFO patients are more likely to have white matter lesions and abnormalities in brain functional networks. Although PFO closure has demonstrated therapeutic effects on migraine in some case studies, the results of randomized controlled trials have not been satisfactory,suggesting that further validation of its therapeutic effect is still needed
