摘要:
导管射频消融是心房颤动治疗中节律控制的主要方法之一,但在治疗持续性心房颤动中维持远期窦性心律方面仍有待提高。Marshall韧带因其特殊的解剖位置和其与心房颤动的相关性成为当下的研究热点。近年来临床研究表明,Marshall静脉无水乙醇消融联合导管射频消融(射频导管消融?)策略在持续性心房颤动治疗中显现出良好的临床效果,提高了术后远期窦性心律维持率,同时并未增加并发症发生率。现就Marshall韧带相关心律失常发生的解剖学、生理学基础以及Marshall静脉无水乙醇消融的治疗机制、可行性、安全性、有效性以及并发症等方面进行综述。
Abstract:
Radiofrequency catheter ablation is one of the primary methods for rhythm control in the treatment of atrial fibrillation(AF),but it still needs to improve to keep long-term maintenance of sinus rhythm in therapy for patients with persistent AF. Ligament of Marshall has become a research focus due to its special anatomical location and its correlation with AF. In recent years,clinical studies have shown that Marshall’s intravenous anhydrous ethanol ablation combined with radiofrequency catheter ablation strategy has shown good clinical efficacy in the treatment of persistent AF,improving postoperative long-term sinus rhythm maintenance rates without increasing the incidence of complications. This article reviews the anatomical and physiological basis of Marshall ligament related arrhythmia,as well as the treatment mechanism,feasibility,safety,effectiveness,and complications of Marshall intravenous anhydrous ethanol ablation
