摘要:
目的 原发性干燥综合征(pSS)患者合并肺动脉高压是一个临床少见却很重要的问题。现主要探讨血清糖类抗原125(CA125)水平与pSS肺动脉收缩压(PASP)之间的关系,并评估CA125作为pSS的PASP的潜在生物标志物的可能。方法 采用横断面研究,纳入2008—2020年于成都市第三人民医院住院的成年pSS患者。收集患者的临床资料、实验室检测指标,并采用超声心动图评估PASP。采用多元线性回归分析CA125与PASP之间的关系,并利用限制性立方样条模型和机器学习算法进一步分析。结果 共纳入126例pSS患者,发现CA125水平与PASP呈正相关(r=0.24,P<0.001)。多元线性回归分析显示,在校正多个混杂因素后,CA125仍是PASP的独立影响因素(所有模型P均<0.05)。限制性立方样条分析提示CA125与PASP之间呈线性关联。机器学习方法显示CA125是PASP的重要预测变量。结论 CA125可能是判断pSS患者发生肺动脉高压的潜在生物标志物,监测CA125有助于早期识别和诊断pSS相关肺动脉高压。
Abstract:
Objective The co-occurrence of pulmonary arterial hypertension(PAH) in patients with primary Sj?gren’s syndrome(pSS) is a clinically rare but significant issue. This study primarily explores the relationship between serum Cancer Antigen 125(CA125) levels and pulmonary arterial systolic pressure(PASP) in pSS and assesses the potential of CA125 as a biomarker for PASP in pSS. Methods This cross-sectional study included adult pSS patients hospitalized at Chengdu Third People’s Hospital from 2008 to 2020. Clinical data and laboratory test indicators of the patients were collected,and PASP was evaluated using echocardiography. The relationship between CA125 and PASP was analyzed using multiple linear regression analysis,and further analyzed using restricted cubic spline(RCS) models and machine learning algorithms. Results A total of 126 pSS patients were included,and it was found that CA125 levels were positively correlated with PASP(r=0.24,P<0.001). Multiple linear regression analysis showed that after adjusting for multiple confounding factors,CA125 remained an independent influencing factor of PASP(all model p-values were less than 0.05). RCS analysis indicated a linear association between CA125 and PASP. Machine learning methods showed that CA125 is an important predictive variable for PASP. Conclusion CA125 may be a potential biomarker for the occurrence of pulmonary arterial hypertension in pSS patients,and monitoring CA125 may help in the early identification and diagnosis of PAH in pSS.
